In our Mapping the Future Family Meeting an aged care nursing professional will meet with you and your loved one in their home at a time convenient to you. They will identify your family’s and loved one’s needs and care requirements and help you navigate the complex maze of private, government and charitable agencies, as well as eligibility requirements, services and benefits available to you and your loved one.
They will also provide you with an overview of the various legal, financial, and emotional obstacles which your family will need to address, and, with your consent, arrange for referrals to our trusted legal, financial, counselling and mediation partners where appropriate.
There is nothing like the comfort and security of the family home and as aged care nursing professionals, we understand the value in providing care for your loved one in their own home.
As Home Care providers ourselves, we can arrange for immediate in-home nursing care within 24 hours if required. We can also assist in arranging government funded or subsidised HACC (Home And Community Care), CACP (Community Aged Care Packages), EACH (Extended Aged Care at Home), and EACHD (Extended Aged Care at Home with Dementia) in-home care packages to further meet your loved ones’ care needs and keep them at home longer.
Many of the medical incidents that lead to our loved ones entering care facilities or hospitals often could have been avoided if the family were aware of the signs and symptoms.
Our full day Carer Training Sessions are tailored to address your loved one’s particular care needs (e.g. sleeping, mobility and toileting problems) and educate you on the most common incidents that occur with the elderly. We provide advice on how you can prevent them, how to administer a variety of medications, utilise correct manual handling techniques, and develop strategies to prevent ‘carer fatigue’.
Choosing an aged care facility for your loved one can be an overwhelming task. Especially when combined with feelings of guilt and abandonment that often accompany this stage. Whilst the selection of a facility is typically based on availability and location alone there are many other factors that need to be taken into account to ensure that your loved one’s care requirements and lifestyle preferences are best met.
Our Finding the Right Home for You Placement Service is run by aged care specialist registered nurses who have experience working in and managing aged care facilities throughout South-East Queensland. Our nurses will meet with you and your loved one in their home at a time convenient for you, discuss your loved ones’ care needs, address any current issues and fully explain the process of entering into aged care facilities.
They will assist you to work out the most important attributes in a facility for your loved one from a clinical perspective, whilst also taking into account your loved ones’ personal requirements. If you choose they will accompany you on facility visits and/or work with you to narrow down and rank facilities based on their ability to meet your loved one’s care requirements and lifestyle preferences.
We can be with you on admission day to ensure everything runs smoothly and will conduct regular post-admission reviews with you and your loved one to ensure a good match between the facility and your loved one has been achieved. We can provide as much or as little assistance throughout the placement process as your family requires. We can give you occasional advice or manage the entire process from start to end. We aim to make the transitional process as easy and stress-free as it can be for all involved.
